Add a signup form to RE NXT. Include phone number in the options to include on the form

Minimally, such a signup form should include name, phone (optional), zip code. What's really needed is a intake form else how are we supposed to get people into the database to cultivate? All RE forms are donor-centric. That's a big problem.
