Include Campaign code and Fund code as fill-in options within Actions

We use an action called gift notification to help us identify pledges attached to proposals.  We also want to see the campaign and fund code for proper pledge identification.

    Having Fund and Campaign on an Action would make it much easier to associate action records to specific fundraising initiatives, which is something our leadership wants us to be able to do.
