Add Tribute info merge field for NXT Online donation confirmation emails. Merged

This is the biggest single hold-up for us switching to NXT online forms. We have the tribute module for Database view and when someone donates through OLX, they get a summary of the gift details (ie: who it is in memory of and who the acknowledgment will be sent to) We do not want to give that up.

    Summary gift details should provide tribute name, who should be acknowledged along with providing a space for the donor to include any other pertinent details (i.e. gift give on behalf of there Sunday School Class, etc.)
