Default adds of Primary Addressee and Salutation when Adding a new record

This should also add the default value when a record is added through batch, forms, import, or any other method of adding a record.

Automatic Creation of Addressee and Salutation Merged

Have new donors created automatically through online forms get a Name Format of Addressee and Salutation. This is a required field for us, but RE NXT bypasses this requirement when constituents are made automatically via online forms
