Include option for tribute gift in RE NXT form designer Merged

Currently, the RE NXT form only has the option for the gift to be anonymous or a corporate gift. The form creator in Raiser's Edge database view allows you to create forms with a tribute gift, include the memorial or honoree, and includes a comments section.
