Add Volunteer Emergency Contact and Medical details to NXT

This is key information for Volunteer Managers / Coordinators when out at events, especially if someone was taken ill or there was an accident, we do not appear to be able to access RE7 via NXT when using an Android so we currently carry paperwork.
