Custom Field Tables - allow to be a dropdown on donation forms

Now that custom fields with a data type of "table" are allowed on donation forms, we need the ability to choose how we display those on the donation form.

    This exists today and is based on whether the table field is setup as "Only allow one per record" or not.

    If the table is setup as "Only allow one" then the field will display as a dropdown.

    If the table is setup without selecting this, it will display as a checkboxes so that the user can select multiple.
