Add Dynamic Custom Fields to Donation Forms

Would like to see the fields change on the donation forms based on the level of giving that a donor has selected to include Thank You Gift Options. Would also like to be able to set up the one-time giving and monthly giving forms differently (one-time would not necessarily receive a Thank You Gift, and Monthly would). And would also like to include pictures next to the options. (see attachment for example)

    This is basic functionality that most online form builders have. It's truly a disappointment that Donation Forms are missing such a crucial feature.
