Give the option to make responding to consent mandatory

At the minute you cannot make the response to a request for consent mandatory. If someone does not select Yes or No then it returns a "No response" in the consent field. Because GDPR requires "explicit consent", if someone does not respond this amounts to declining consent. If a volunteer has previously given permission, but then fills in a form that asks for consent and does not respond, their previous permission to contact is overruled.

    This feature is neccesary for mailing list sign-up constituent forms too, otherwise if the user do not select a category of consent for any reason, it still creates a record without consent and the user will receive the confirmation email of being subscribed, but then won't receive any of the mailing list emails. For GDPR reason, we can not mark the consent categories activated by default, so we need to make sure the user have to select consent before submitting the form.
