Add Campaign to Online Donation Form (RE NXT)

RE NXT's Online Donation Form allows you to select the Fund and Appeal, but not the Campaign. This is a glaring omission! Campaign is essential to our database setup and health, and I'm sure it is for others as well. Please add Campaign to the Online Donation Form!
