Web view donation forms need option to associate to an Appeal's Package Merged

One use of Package is source, like acquisition list. If we send different sources to different pages, we need to know that. They're not different appeals, they're different sources which are different packages. We need the donation form to allow that.

    I see several suggestions getting at the same basic idea: we need to be able to track from which packages donations are coming, and donation forms should have that functionality built in.

    When testing the landing pages and reviewing the donor files, we discovered that the package arrays do not populate, and per the team, we would need to input those manually. This is a hugely labor-intensive process, as we could have thousands of donations, creating a hardship on an already time-strapped team.
