Add Class of and Constituent Code years options to Constituent Forms

It would be really useful to be able to have 'class of', and school years, so constituent code 'date from' and 'date to' on constituent forms as our alumni typically have an affinity to their yeargroup and this knowledge is important to relationship building.

This would also be helpful for constituents to have a "maiden" or "former" name field so so we can match correctly.
