The current Constituent forms apply updates directly to records, but ignore most of the fields that provide an audit trail of changes made. At a minimum the form should:

• Add Valid from date when it is adding a new postal address

Ideally it would also:

• Add an Info Source for new postal address (not currently available in web view)

• Change existing address to type 'Previous'

• Add 'Valid to' date to the previous address

• Add an Action (or Email History) to show that the constituent filled out a form on a particular date
