Make Title 2 and Suffix 2 fields always visible when adding or updating individual/guest records.

Problem: Currently, these fields only appear after Title 1 or Suffix 1 are populated, which limits flexibility.

Use Case:

• Title 1 = Dr., Judge, etc.

• Title 2 = Mr., Mrs., etc.

• Suffix 1 = Jr., Sr., etc.

• Suffix 2 = MD, PhD, etc.

This setup is needed for address/salutation variations and to work around system-wide Name Configuration settings in Web View.
