Add the ability to apply the UserWay accessibility widget to Optimized Forms

In order for Optimized Donation Forms to grade above 60 in accessibility grading tools, a UserWay accessibility widget needs to be added to the form. Currently, this feature is unavailable. Please enable this feature to ensure ADA compliance is being met to the best of everyone's ability - thanks!

    Every educational institution is required by law to be ADA compliant. Blackbaud needs to ensure that every aspect (esp. donation forms) and ALL components of NXT and Database view can be easily compliant with ADA.
