Add the ability to set the campaign on Optimized Donation Forms

We like the idea of the optimized donation forms but don't want to go backwards in terms of functionality. Everything currently available on NXT donation forms should be available on the optimized donation forms

    We'll be adding in a LOT of features to optimized form later this year - while there are certain features that don't "fit" within the optimized model most features will be available on both forms.

    We do already support adding an appeal on optimized forms. I am going to change your title for just campaign so we can see voting on that specifically.
