Gift Aid on new NXT Forms

Is there any way that we can have boxes on the Gift Aid Section of the NXT Forms that covers - no to this gift but my current declaration for other giving stands

      From my perspective, it would be best to have a separate bespoke form for collecting Gift Aid, which then updates a constituent's declaration, rather than on each individual gift.
